I wish to enroll my child ['the Applicant'] for the 2012 Ottawa FURY Summer Camp ['the Program']. We agree to pay the full fees for the Program and have read and understand the Refund Policy [listed below].

Refund Policy
The Ottawa Fury Soccer Club recognizes and appreciates the need to service participants by providing a vehicle to obtain refunds in the event of a conflict. The club shall reimburse 100% of the registration cost if the request is made more than 10 business days prior to the start of the program. Requests for a refund made less than 10 business days prior to the start of the program shall be reimbursed at 50% of the amount paid. No refunds shall be given for requests made after the participants' first scheduled program session or practice. Genuine documented illness, injury or emergency situations will be excluded from the above policy and refund eligibility will be determined on an individual basis. Please contact Graeme Ivory (graeme.ivory@gmail.com) to discuss your situation. To qualify for a refund, the following criteria must be met: 1. Requests must be made in writing to the club via email (info@ottawafury.com) or by mail (458 MacLaren Street, Floor 2, Ottawa, K1R 5K6). 2. The date of the request shall be the date of the email or the postmark date of the mailing. 3. The request shall include the amount requested, the name of the player as well as the parent's name and address for payment. 4. Once received, the request shall be approved and forwarded for payment processing. 5. Applicants should allow approximately 60 days for processing.

In consideration of the Ottawa Fury Soccer Club L.P. ('the Club') accepting the Applicant into the Program, I, the parent/guardian of the Applicant, for myself, the Applicant, any other parent/guardian of the Applicant, our heirs, administrators and assigns hereby (1) acknowledge that there are numerous inherent risks associated with the Applicant's participation in the Program and that I assume complete responsibility for those risks and for personal injuries, damage to property, accident of any kind, or death that may occur as a result of those risks even if the injury, damage, accident or death occurs in a manner that is not foreseeable (2) waive any claims, demand, damages, actions or causes of action (collectively called 'Claims' below) to which any of us may be entitled, arising out of or in consequence of the participation or attendance of the Applicant in the Program and any loss, injury or damage to my person or property or to the Applicant's person or property incurred while the Applicant attends or participates in the Program, and (3) releases the Club, its owner, shareholders, directors, servants, agents, sponsors, employees and any other person or organization assisting in the program (all of whom are collectively called the 'Program Providers') from such Claims, notwithstanding that any such loss, injury or damage may have arisen by reason of the negligence of all or any of the Program Providers.
